Liaison Specialist Practitioner

We are seeking an enthusiastic, adaptable and patient-centred mental health practitioner to join our Liaison Psychiatry Service. This is an exciting opportunity to work at the interface of mental and physical healthcare, supporting patients across the Leeds Teaching Hospitals NHS Trust

The successful candidate will provide specialist biopsychosocial assessments for working age adults and older adults in the acute trust hospital. The role currently includes assessments for children and young people presenting to the emergency departments with mental health needs.

We are looking for an individual who embraces change, is forward-thinking and solution focused. You will be a strong team player with excellent communication and leadership skills, able to build effective working relationships across professional and organisational boundaries.

The role requires a clinician who can think beyond immediate presenting problems, appreciate the wider healthcare system, and contribute to the ongoing development of integrated mental health services within acute care settings.

This is an exciting time to join the service as liaison psychiatry and emergency mental health pathways continue to develop. We are seeking someone who is comfortable working within a dynamic environment, can adapt to changing service needs, and is keen to contribute to service improvement and future models of care. As local and wider healthcare services evolve, the responsibilities and focus of the role may develop over time to best meet the needs of patients and services.

The Post holder will:
Provide specialist mental health assessments and reviews to medically admitted working age and older age adults referred from Leeds Teaching Hospital Trust with co-existing mental health and physical conditions.

Be the first point of contact out of hours for mental health in reach referrals from Leeds Teaching Hospitals Trust.

Provide a mental health assessment to patients under the age of 18 (Children and adolescents ) who present to the Emergency Department (ED) out of hours with self- harm or mental health difficulties

Triage, prioritise and coordinate mental health assessments through the appropriate care pathway.

Provide clinical support for teams across the mental health inreach teams and the Acute Liaison Psychiatry Service.

Work in a 24/7 roster including Out of Hours.
