Job Requirements/Description

With good practice guidance from the Team Leader, work as part of a team to enable the people we support to live their best quality of life, to remain safe and happy, leading a fulfilled life of meaningful opportunities.

Community-based.

This role will be based at Croft Community in Malton.

Using person-centred approaches, work as part of a team to promote the rights and choices of the people we support, whilst maintaining their health, safety, dignity and privacy, thus enabling them to lead a meaningful life, whilst also empowering them to be as independent as possible.

Main responsibilities:

  • To comply with the Trust’s Policies and Procedures, including Safeguarding, Equality & Diversity and Data Protection.
  • To comply with the statutory provisions of the Health and Safety at Work Act 1974.
  • Employees must look after their own health, safety and welfare and be mindful of other persons who may be affected by their acts or omissions. Employees must co-operate and comply with management instructions regarding Health and Safety issues and report all safety events and safeguarding concerns as soon as practicable to their manager or other senior members of staff where necessary.
  • To ensure that confidentiality is respected and maintained at all times.
  • To attend and participate in Team Meetings, 1-2-1 Supervision sessions and other meetings as required in line with the post.
  • Working under the direction of the Team Leader, you will plan and provide support and care to promote the wellbeing and development of the people we support in accordance with their person-centred support and care plan.
  • To work co-productively on the support plan, recording and reviewing progress as directed, so that the people we support can fulfil their potential for independence, choice, inclusion and control.
  • Encourage the people we support to maintain a clean, tidy, and welcoming living environment.
  • Support to use appropriate laundry equipment so they can be enabled to do their laundry and clothing.
  • Plan, shop and prepare healthy and wholesome meals alongside the people we support considering their health, choices and wishes.
  • Encourage the people we support to be healthy by assisting them to arrange and attend medical appointments.
  • Enable the people we support to look after their finances by supporting them individually to budget, monitor their finances and access benefits.
  • Support and encourage the people we support to maintain an active and supportive social life considering individual choices and wishes.
  • Provide personal care where required and appropriate as a part of their support and care plan (e.g. bathing, hair washing etc) and where the appropriate registration is in place. Please note that this may involve supporting people to the toilet, to manage incontinence, to bathe and shower etc and enable people to manage all of this, with regard for their dignity and privacy.
  • Administering and recording of medication, order/check/count medications.
  • Co -productively plan menus based on their individual preferences and diet choice, making a shopping list, preparing the food, cooking the food, safe handling, and storage of the food.
  • Related paperwork includes support plans, daily updates, person profiles, risk assessments, incident reporting, behaviour reports and notes/updates on health appointments.
  • Ensure tenants receive their housing related support by enabling tenants to look after their finances. This includes developing appropriate budgeting and financial training including support to access benefits and maintain them and assist them to understand and maintain their tenancy. Assisting tenants to access services from the landlord.
  • Work within the community’s policies and procedures and code of conduct, actively promoting the equality and diversity and the human rights of tenants to realise independence, choice, inclusion, and control.
  • Undertake training and continuous professional development in accordance with The Care Quality Commission, local authority requirements, the needs of the service and the people we support.
  • Work in close partnership with family members and other professionals involved in the individual’s life.
  • Undertake personal development to progress and gain an understanding of our philosophy and values and how these inform and guide practice in the key responsibilities of the role.
  • Promote every individuals quality of life and the use of various communication methods used by the people we support.
  • Complete actions from audits under the supervision of the TL.
  • Enable choices, healthy lifestyles, positive relationships, spiritual fulfilment (this will be different for everyone) and meaningful activities for the people we support.
  • Complete daily monitoring sheets such food, fridge, freezer, medication cabinet temperatures, bowel charts, weight charts and fluid charts etc.
  • Have a positive attitude and enthusiasm for the job we do.
  • Respect the community and the people who live and work here.
  • Be flexible and adaptable according to the needs of the people we support and be expected to work anywhere on site as requested by Team Leaders and Management.
  • Report any maintenance issues to the PALS team utilising the current on line system and maintain the maintenance log.
  • To undertake any other duties which are consistent with this post, as directed by line management.

Education & Qualifications:

Essential.

  • A basic understanding of English and Maths in relation to writing daily notes and supporting with finances.

Desirable.

  • Care Certificate.
  • A level 1 or 2 in a health and social care qualification.

Knowledge & Experience:

Essential.

  • Be proactive and forward thinking.
  • Be empathic, kind, caring and a good listener.
  • Willing to learn new skills and ways of working to enable the people we support to live their best life.

Desirable.

  • Experience in working with technology and/or digital platforms to upload data such as daily notes.
  • Driver with clean licence.
  • Some experience of working with people with a learning disability and/or autism.

  • Be adaptable to the changing needs of the people we support as well as changing legislation, policies and procedures.

  • Promote every individuals quality of life and encourage the use of various communication methods used by the people we support.
  • Work as a team to ensure that the people we support remain safe, happy and healthy at all times, following support plans, risk assessments, task analysis, SMART targets.
  • Be a good problem solver and have a practical approach to being part of a shift.
  • Work as part of a positive team but also as an individual who can lone work.
  • Support newer staff in a professional way and offer advice and support on code of conduct and good practices.
  • Be able to remain calm in stressful situations and respond appropriately to any challenges or situations that may occur.
  • Understand the limitations of the role and when to seek support from more experiences staff and those in a more senior position.
